Emergency Contact (Someone outside of the home)
Your child will be cared for as if he/she were our child. While there are some inherent risks involved with any event, every precaution is taken for the safety and good health of your child. In the event of accident or sickness, by signing below, I am releasing Vitalpoint Church, directors, employees and volunteers from all legal liability, any and all actions, causes of action, claims and demands resulting from any loss, injury or damage to person or property with respect to events (including overnight) or gatherings organized by Vitalpoint Church.

In the event that your child requires special medication, x-rays, or treatment, the parents/guardian (as indicated below) or the emergency contact person will be notified immediately. Your child must be covered by Provincial Health Insurance or equivalent medical insurance. In case of medical or surgical emergency, I hereby give permission to the physician selected by a representative from the organizers to hospitalize, secure proper treatment for, and to order injection, anesthesia, or surgery for my child as named above until the parent/guardian (as indicated below) can assume this responsibility.
Dismissal & Behaviour
I understand and agree that I am solely responsible for my child’s behavior and that my child will obey all the rules and regulations pertaining to Vitalpoint Church Events/Gatherings or risk being sent home, with no refund. I further understand and agree that I will solely responsible for arranging transportation home. I understand that organizers and its directors, employees and volunteers are not responsible for any injury, loss, or damage of any kind sustained by my child or other participants. Further, the organizers reserve the right to dismiss a child, without a refund, who is deemed to be a hazard to the safety or rights of others. The organizers further reserve the right to search my child’s belongings for prohibited items, if suspected.
Cell Phone Policy
All cellphones should be “off and away” unless students are otherwise given permission at specific times by a VPC Student Team Member. Cellphone use will not be permitted during non-instructional times including during social times, at events or during nights that don’t include a formal lesson. 

Our vision is to cultivate a culture where students are given the opportunity to explore and grow in their faith as well as spending time with one another. We strive to ensure that we are making the most of our few hours each week together by offering a distraction free environment for each student and team member and giving our full attention to God and one another. Along with our desire to build relationships, we also want to keep this space a safe space for everyone.

If anyone is caught using their phone they will be asked to put it away and if caught again on the same night they will be asked to give it to their leader until the end of the night.
Photo Release
I grant permission to the organizers and to any third party authorized by the organizers to use photos, videos, or any other recording or reproduction of the participant in any medium for use in promotional materials and/or as otherwise seen fit by organizers.

By signing below, I acknowledge that I have read this form in its entirety and understand and agree to the above statements and, further, ensure that the information given is complete and accurate. I hereby consent to allow my above-noted child to participate in events/gatherings organized by Vitalpoint Church.
